Nurse’s Office

A registered nurse is present in the Saint John XXIII Nurse’s Office during school hours to provide a safe, caring, and healthy school environment and to address students’ social, emotional, and physical needs. Emphasis is on prevention, comfort, and health promotion to foster success in school and ongoing physical and emotional well-being.

Immunization Information

Arizona State Law requires that children must have proof of all required immunizations, or a valid exemption form completed, in order to attend the first day of school.

Please check your child’s immunization record with your child’s doctor and provide proof of a completed shot record for your child. If your child is entering Kindergarten or 6th Grade chances are your child will need to have vaccines.

** Kinders need DTaP, MMR, OPV, and Proof of Varicella vaccine or history of having Chickenpox disease.

**6th graders need Tdap, Meningicoccal, and Proof of Varicella vaccine or history of having Chickenpox disease if they are 11 years old on the first day of school.
